I do it by mouse alone, I found that there's no need to be overly detailed. The amount of loras I use take care of it pretty good.

Experiment with the amount of denoise and steps. A single point in denoise can be massive. For my setup, if I swap from 69 to 70 denoise, it's night and day. 7 steps from 10 also changes massively.

Most difficult part is to get the style you want, I often end up with soft blurry background when I get the style I want, so I have to do extra work to get the background.

In the scale field, try using 'resize to' rather than 'resize by' and use a standard SDXL resolution (1024x1024, 896x1152, etc.)
Assuming your inpaint mask is only on the area you were sketching, the other pixels should be left behind properly.
If you're applying img2img on the whole image, you'll expect some stuff to get lost (especially since in this instance you're not prompting for much that's related to the rest of the image).

My guide was primarily written for people to clean up gens they were making from scratch - it's not quite the best set of instructions for editing other images (you'd probably want to use the sketch tab to add a general shape/color of the added bits you're after, and then use a tight masked inpaint to change the blob into what you're after).

I think the biggest issue I'm having right at this moment is the base image was pretty low res and fitting in detail cleanup through inpainting isn't as successful as I'd like because of it.

I decided to toss it in img2img with as much of the original prompt as possible and upscaled it by 1.5 from 848x1120 to 1272x1680 and re-touched it up all over again.

Negging the undesired type of piercing and 'piercing' in general can help stabilize the gen too imo. The reason I used that model is that it's a fairly neutral model (similar to 102d vpred) that deals well with style/artist tags but has a greater bias towards e621 for more precise ball-size control (as seen on that goblin sequence) and better animal genitalia.
It does have a slight realism bias but that can be managed by other style tags or stamped out with negatives. You may also be interested in StableMondAI-SDG, which is an older, more 2d-focused version of that same model.

I'm surprised how popular futa twins showing their butts off was in this thread.

i also spent an unhealthy amount of time seeing what kind of dick sizes i could achieve within a fairly strict style lora
and just cranking the intensity of the dick tags until the pictures broke
right there is the technically largest cohesive one i got
given that it's literally grazing the ground

there's kind of a dance i can do with the model where
i put can try to balance keeping the dick in-frame while ratcheting up the size and try not to let the entire thing fall apart completely

i want to share most of them but i'd like to clean up the fucked up eyes and surrounding details to make them a little more palatable
because if you enjoy any kind of hyper, a lot of these came out pretty cool in that direction

It's 100% WAI. It's specifically trained to have a very normal anime look and I'm pretty sure there's little to no hyper in its dataset. That style is part of why this Lora is so strong on Noob. Honestly, I'd recommend switching to another model when you want to do hyper.

Here's a rejected snake. The base prompt is completely intact on this, though I've used different artists at different strengths and altered the posing/penis detail. Usually I just use this as a starting point and make heavy use of inpainting to get what I want though. The takeaway is that "outside" + "cave" gives really neat backgrounds.
The two first cows are also mine. There's no real box for them; I just genned a multi-breast futa and edited in the second dick, then inpainted both of the dicks to have milking cups on them and fixed up the existing nipples and the faces to look coherent.
