Sorting out nutrition, intermittent fasting, and big 6 compound lifts/movements (squat, dead, bench, row, overhead press, pullup/cable pulldown) at any resistance level, freeweight or machine assisted, would be vastly more helpful to obese patients but you can't charge $1000 a month for groceries, vitamins, and a planet fitness membership.
