I feel like that's hard to answer. It seems like there are a lot of shameless fetishists who just get boners from wearing a skirt and using women's bathrooms. But even those of us who aren't like that tend to have sexual fantasies, and in those sexual fantasies imagine ourselves as women... And we hide them and dismiss them as shameful fetishes because of the prevalence of the first group.
