IIRC there were multiple versions. The first version was working perfectly but it was so good at being stealthy (hiding itself and only infecting the target) that they couldn’t confirm if they had actually managed to infect their target. Then they released v2 which was not only more agressive (to make sure the target was indeed achieved) but also bugged and ended up infecting half the world.
They should just have let v1 do its job and it would have taken years for the Iranians to figure out why their centrifuges were spinning out of control.
