How did medieval Europeans (say in the 1200's) have sex and how did it differ from modern times? I specifically mean the act of having sex, not the preparations before and after it. I mean the intercourse.

Medieval sex was pretty similar physically

Same mechanics, same positions.

Just way less comfortable. No modern hygiene, so smells and infections were common. Women had zero rights, so consent was shaky. Contraception? Mostly guesswork and luck. And forget foreplay. It was quick, utilitarian, and male-focused. Romance? Rare unless you were nobility.

Commoner women could marry older, it was mostly nobility and rich people who got their daughters married off young. Women were workforce in farms so it is not like peasant families were necessarily in any hurry to give their daughters away.

Yeah the western medieval/modern model (ever since the advent of manorialism pretty much?) has been men and women have their fun, if they get too carried away and/or someone gets pregnant then they get married. Also marriages were seldom arranged that was usually reserved for the nobility (and even then they had a culture that stressed the importance of love and at least nominal consent especially after the 1100s)

Well the Catholic Church changed the law and made it so women could marry unilaterally even against the permission of their fathers (marriage was really the only legal domain the church had completely authority over everywhere). If she could land a man who didn’t want a dowry though, then he would probably be fine with it, but the upshot is that Vegas style weddings in response to pregnancy were very common unless the dad was noble and could afford having bastards and financially supporting an illicit family. Sometimes even priests did that, Rabelais was a lifelong live-in gf he fathered children with, essentially a common law wife despite being unable to legally unionize since common law marriage wasn’t a thing yet
