Most Egyptian cities don't even have a public transportation system dude the plebs either get around by train or private individual owned and unorganized microbuses that have fuel subsidies from the gooberment in return for keeping prices somewhat low for public consumers but that's it. We're not just underfunded, our transportation system gets no funds because it doesn't exist in most cities beyond trains.

This is part of it. Regan shut down the asylum system a few decades ago because muh lobotomies and muh abuse. So I stead of fixing it they just kicked everybody out onto the streets lol. Public transit in the US is very hit or miss. Some places have clean trains and no nut cases--the DC metro was surprisingly clean. Some places have subway systems with piss and shit soaked cars and people ODing ontop of you (NY, SF). In most places like where I love in NH it just doesn't exist.

Mental illness and random violence are hallmarks of the American experience though, one way or the other. It gets exaggerated by some but there are lots of thirdie tier locations here which makes their juxtaposition against neighborhoods next to them that are the richest in the world all the wilder.
