I understand it's hard to get for people who aren't or haven't been in a similar situation. It's just, these weren't ambiguous feelings, it started with mentally concluding that "I'm a girl inside" almost verbatim at a very young age and carrying that inside forward. Never stopped thinking about it, hating puberty while wishing I'd somehow turn into or wake up as a girl, getting really excited at every chance to present as or engage in something I felt was feminine or female-coded. I get that there are people who are just generally depressed or unhappy with their body because they're ugly or short or whatever and use dysphoria as an excuse/cope, but for me it really did just start with a female identity and unhappiness in this derived from increasingly obvious incongruence between my body and life and that identity.

It is simply connected to an apathy of sexual identity. I was always just me and didn't feel any attachment to womanhood or manhood.
But due to the way I was born I was defaulted into manhood and struggled immensely with living up to it.
By the time I was ten I found myself surrounded by female friends, attracted to males, wearing jewelry and being admittedly a bit weaker both physically and mentally than other males around me, yet I did not associate this with being a woman, I was simply a weird faggot.
Only much later in my 20s did I realize that if I COULD pass as a woman I would have a much easier time being myself and having people around me understand better who I am. THAT is why I transitioned.

It can be difficult to differentiate between male and female adults. This is even more pronounced the earlier into puberty someone is.
In these case where they don't even register their body as a male body, the only dysphoria would come from social pressures to adhere to what they see as a completely arbitrary, pointless, and discriminatory set of rules(this never gets resolved). So someone isolated or with a lax family could easily not experience too much disgust with their gender because they don't even associate with it in the first place. They just end up in this weird limbo of not thinking about it which is simultaneously what they're supposed to do and a transgression that will see them outcast from their family and society. So begins the tranny dissociation.
