Phantom Pain are loyal to Djbril first and foremost, so if Neo refuses his orders, Djbril orders one of the other guys either to shoot him and then reprogram Stella to see someone willing to carry out his orders as her new mentor figure or has his other guys hold Neo down and reprogram him instead to be more willing to obey his orders. Remember Neo's has his brains fucked with too, even though he wasn't made an Extended. And if Neo somehow manages to get to an MS fight off the whole squad and escape with her then Stella is now cut off from her maintenance pod again (which isn't exactly something he can take with him) and is dying again. There was zero chance of her getting the maintenance to live if she didn't fight again, it's not like Djbril was gonna expend resources to keep an Extended alive that wasn't fighting for him, so Neo was kind of stuck in a shitty situation.
