Humans are somewhere in between, but actually closer to elbows out because your two forearm bones are at a slight twist when relaxed and human arms aren't straight, they're slightly bowed at the elbow.

Stand relaxed. Now put your elbows facing away from your body. Twist your fist so the back of your hand is also facing out. Feels bad right? Now do the same but have your elbow back. Doesn’t make you feel like a tard right? That is your answer.

For non-Zakus, does the shield mount to the side or the back of the elbow? That's generally how I make a decision.
