Is it true that the classical composers heard the music in their head before writing anything down or even playing it?

If you're that well practiced, maybe. But most people can think of a tune in their head. It's not that extraordinary.

the usual approach is mindless noodling for hours on end sat at a keyboard until you come across something you like, this is how Mozart and Bach did it

Right, my head know what sound I am going to get out of my instrument if I place my fingers thus. In my head I know how to control this instrument. That I haven't heard it yet doesn't make a difference. Similarly when we talk, we don't write out 50 drafts first.

Possibly not, but it is do-able. I can hear a noise in my head and then go and play it. But it's more normal for me to play it first. Why? It's louder and then other people can hear it.

It's usually unhearable but yes it's in their heads first. It's half right. Most of the time. I have composed purely from hearing sounds in my head and knowing internally what intervals they were and then written it down, but usually I have an instrument handy.
