Have you 100% used a completely closed back square back ideally S marked Schmid tool mil spec trigger? Low shelf lower? M16 BCG?
Also at this point there is the possibility you've bent your lever out of spec. Happened to me. once it is bent out of spec, it won't reset any more. Inspect carefully. If there are signs of cracking or wear on the top convex face of the lever, that could indicate possibly it has been bent out of spec.
My first Atrius lever wasn't really working, I was trying to force it into various combinations of lowers and uppers. It eventually bent, and stopped working entirely. I got a replacement, and just hit 800 malfunction free rounds yesterday.

Sorry correction, that image with the cracking was my first lever which bent and cracked within 120 rounds or so, but I believe I caused it to bend/crack by initially trying to get it to work in an 80% receiver that had been finished to 100% out of spec by myself on a hobby mill.
