Did that ever happen? The Imperium sent 14 million, but the defenders were 8 million. That's less than half the ideal

Vraks barely had any economy, or civilians. It was basically all military storage. Essentially 7.5 million PDF.
Not full guardsmen, but not "2/3 are women or children" either. And in some imperial planets, women and children are trained to PDF standard – just look at Cadia or Catachan.

I guess that's the imperial standard + the Survivor's fallacy: the only forts standing today are the ones that were hardy enough
