The number one worst thing for efficiency, besides malformed 'crash safety' tests/regulations that simultaneously push cars to be both pigfatter and yet also disintegrate like tissue paper at the same time, is the fact that nitrous oxide was defined as a pollutant despite being non-toxic, which puts a hard cap on how high combustion temperatures can get, which in turn puts cap on how thermally efficient the engines can possibly be. (And, as an aside, also increases how much particulate smog and volatile hydrocarbon emissions there are in the exhaust.)
In other words, the law of the land is that engines have to be less efficient, not more, actually, and this is a Bad Thing(tm).
