When I used to work IT there were like 3-4 managers in charge of our knowledge base/procedures handbook and they would purposely leave shit out or put in wrong directions just so they had something to correct the new hires on and write them up for, I remember first time I came across it, there was an app that wasn't working and in the handbook it said "DO NOT UNINSTALL AND REINSTALL THE APP OR RUN APPFIX.EXE THAT IS PUBLICLY AVAILABLE IN THE TOOLS FOLDER IT WILL BREAK IT AND YOU WILL BE WRITTEN UP!" in bold as well, so after fucking around forever I ask how to fix the thing because I'm at a dead end and the fucking mangers all at once tell me "oh yeah reinstall the run the fix" I ask why this isn't documented and they just say there's no reason to update it you'll learn if you stay with the company long enough, meanwhile the guy I'm helping is pissed and throwing a fit because I wasted an hour of both our time for a fix that took less than five minutes
