Many moons ago during a game of 4th edition my opponent had Eldrad Uthran in his army, and 3 turns in a row he rolled 2, 2, 12 on his psychic tests. So, the most venerable and powerful psyker in the setting shows up to the field in all of his finery and then eats his own head and explodes.
On a related note I once lost a 10 man Terminator Command Squad to a failed Deep Strike roll. Welp.

I've been playing it with a buddy who we both had it sitting in our libraries unplayed for years, it's alright.
