Honestly I think it's a combination of the two.
FRIEND is a parasite and is attached to Dess.
Note how in their first appearance she seems head forward, then tilts it up to reveal a mouth/eye where the neck would be. She's in a trance/dream state while the FRIEND or whatever the fuck has hold of her pilots her body like a puppet.
I'm no drawfag so I can't exactly depict it like I want to so here's a sloppily made comparison using Noelle's face.
