You respawn at any friendly base that still has the facilities to supply planes. The game has an economy going on where you gain certain amounts of cash per minute and get more cash for blowing up enemies and enemy structures. To get a plane you either have to spend your own money on it or requisition it from your faction, but they need to have some available and you need to be a high enough rank in the faction for whatever plane you want. You also have to spend cash on your munitions, you can't requisition those. Usually not a problem, but the game has nukes and those start getting costly, so if you fuck up a nuke run it could be a very expensive mistake.
Game typically ends when all the enemy structures are destroyed and their carriers sunk, if they have any.
