I usually draw the angle and position of the shoulders and hips first, then connect them with the general action line of the spine. Then I add the head.
After that I place the hands, and connect them to the shoulder with arms that make sense in an IK sense.
Then it's just filling out the form.
I also try to map out my shading with this really sketchy nice pencil brush I have as I fill out the form before I add details. It helps me get a clearer handle on the figure.
I still mess up frequently on little details, though, so maybe it's not the best approach.
