Its not just Karui villages but all kinds of islands, people played it back in the Alpha event.
Theres a jungle island with a cave with new siren boss in it.
Theres Flayer Acid Pools island where you get to kill those goblins who have been raping your ears in PoE 1 towns.
Theres a grey undead shore full of broken ships and fat zombies throwing anchors at you.
Theres a flooded prison straight from Dark Souls 2, built by Izaro and has a Skeletor as a boss.
Also the monster mother village from Mercenary gameplay video might be another island (or its in Act 5). Overall Act 4 seems to have most amount of variety and beach-type of level layouts.
