You just need to start small so its not too daunting. Get a shitty part time job where nobody expects much of you so its not stressful then at the very least you will have some structure and time away from home.
I know you are scared to take the first step so just make the step as small as possible without making it pointless, it wouldn't even matter if you barely got anything more than pocket money from the job because you would still be taking the first step in breaking free of the cycle of self destruction you have been born into and raised in.
Just the fact you arent a junkie or dead is already an achievement, don't be too hard on yourself but also don't let yourself be a victim of circumstance. The fact that you have your head screwed on well enough to be aware that continued inaction would lead you to becoming like your folks is a strong indication that you are capable of taking responsibility for your own future and making it out of this.
