Most of these "problems" you're hung up on can be explained very simply in consistent terms.
Worf spine transporter fix? Too complex to modify. In fact that was basically talked about in the show when the crazy doctor was explaining her new machine which had high enough specs to replicate a new spine.
De-aging with transporter? Leads to unstable physiology and degeneration over time if not reverted.
Sci-fi tech is all about have drawbacks and limitations. That's what makes it different from pure magic.
