From what I remember he was generally regarded as a dick even before the leg thing, the leg just made it worse. Real answer is cause he thinks of himself as unlovable so he acts like a turd before people have the chance to reject him.
I never grasped if he's implied to be a savant, handful on Earth type of medical genius (The Good Doctor is more like this) or simply a very highly regarded doctor with a difficult personality. In either case, one would think the hospital would try to uplift and support the man, not constantly question him.
Anyway the show blows ass like all medical or police shows do.

Watch the show, Cuddy has his back more than even Wilson. The whole department is made just for him. She knew he could save lives, but would get fired by any other hospital. From the looks of it he didn't even get paid that well.
