Junior's dementia progression is hard to watch since the people around him don't take it seriously and the chance to try to help him and settle his affairs in good time just passes them by. This is what happens to a lot of old people without families (though he did have the fat retard Bobby). When Tony has his heart to heart and asks "don't you love me?" to Junior, it's already too late for that kind of conversation. Tony's talking to a guy that isn't there anymore and many of us will have that moment with a loved one since alzheimers is so common.
It might be accidental, but Junior crying, but unable to just say "I do" to Tony, is also very real. Dementia makes people more alexithymic so in emotional situations like that they'll confusedly cry, get angry or scared instead of reacting more appropriately. You can't emotionally connect with someone like that anymore. Back when I first watched the scene I thought Junior is just a vain old school tough guy wop who always keeps his guard up so he'd never say "I love you" to his nephew sincerely, but cracks since he really does love Tony. Seeing it again, I think he's just almost fully gone at that point and confused, he feels something strong but can't tell what exactly so he cries. This also effectively tells the audience that Junior does love Tony (later some of his last memories are that he often played catch with Tony), but Tony is unsure since Junior's not clear enough about it.
