You have to suspend disbelief with horror flicks but it was never this consistently bad. Only 10 years ago there was a similar movie where a guy gets glued to a clown costume and the very first thing he did was go to a doctor. The entire movie felt more authentic just because of details like this

Also do they literally just turn into trans at the end?

You could say it's because they were already mindraped by the cave right away but clearly this can't be it given they were still very normal at the beginning and they eventually DID consider going to a hospital, so it makes no sense for them to not consider it sooner.
