you are too used to chillaxing you have yourself trained basically to chillax all day.
you can try doing small things one at a time. every week or every few days or whatever feels easiest. pick one simple thing you want to get done. it could be as small as cleaning your room thoroughly.
as you start getting dopamine hits from getting stuff done you'll slowly start to get more motivated to deal with your problems. it takes time and persistence so don't rush yourself.
