He's essentially a druid. Shamans were considered beyond the normal class of society, not typical citizens and with a higher power that even rulers were meant to respect. Like females in a wolf pack, they're their own class outside the typical alpha/beta/etc hierarchy due to the role they play.
Like regular doctors and in exchange for being looked after by the people, they aid any who come to them in a symbiotic relationship. He has a differing view on life and as such, sees little difference between the normal folks and infected, on top of his hippocratic oath. They're all animals and part of nature, and it's his job to reinforce and teach others this, not just mete out aid. The role necessitates a more detached view. He's like a teacher dealing with a bunch of preschoolers; the preschoolers are in the throes, and he sees how small they are in the grand scheme.
So he's not going to kill the Alpha unless he's in a pinch, because yes, he's there as an observer and knows to let nature do its thing.
