It's the fog of the unknown in the book. It's how you can see the fog but not through it.
I think PTA ended with this scene as a flashback or how Doc copes with the PI life. At the wheel eyeing suspiciousness which he now sees clearly, represented by the headlights, especially hyperaware after the whole Golden Fang case and his attachment/love for Shasta. Just my take
