It's just a metaphor. The guy is cheating so in his mind he has two personas. The spider represents how he perceives commitment to his fiancee. To him she's going to trap him and suck out his life.

People say the spiders represent the way he sees women or whatever but I thought it was more about women being hurt turning into monsters. Like the stripper scene with the baby spider was basically a young girl who hadn't been hurt by the world much yet while the huge spiders walking around the city represented women repeatedly hurt and filled with rage towards humanity. I also assumed his fiance/wife turning into a spider at the end when he walked into the room was due to finding some incriminating evidence like a photo/note he was hiding of another woman which is why she changed so quickly
Did anyone else have this interpretation?
