For all the old school bullshit Phil was talking about, he was a horrible boss and the family knew it, it's why Butchie gave his location up, I also don't think it was New York who killed him or that he died at all in that diner.
I think Carlo flipped when his son got pinched cuz he remembered what Tony said :"fuck that honor and loyalty shit".

There's a small bit where Junior is institutionalized (the episode with the Chinese guy with rage issues iirc) where's talking about how Carmine let a guy get away with fucking his wife because he brought in money, and then had him whacked years later after he stopped earning.
