I literally just began watching it when it was on Netflix back in 2012 and for some reason became obsessed. In the book when he kills Jaworski it happens after he tried to sex Rita and she turned him down, and he was horny, so he decided to kill some random that wasn't on his list. He decides to torture Jaworski which he's never done before. And the entire time Dexter says hes feeling this "pressure" rising and demanding "release" but he couldn't figure out how to release it no matter how much torture he did.

So, yes, he didn't literally cum in his pants, but the torture was turning him on and bringing him to the edge.
