They do occasionally put extra effort in but it's uncomfortable for the actresses and audiences don't care about that stuff. A piece of tape or some cloth tied between their lips is just easy visual shorthand for “gagged”. Going above and beyond is dangerous, unnecessary and makes the director look like a weirdo
