Are there actually clauses in actor's contracts forbidding them from gaining/losing too much weight? Seems unenforceable. Women for example can balloon up out of nowhere because their hormones acted up.

There are a lot of weird clauses in contracts in Hollywood.

In RDJ's case, when he was brought by Disney for the Avengers, his contract prevented him from being seen in public as drunk / druggedm etc since they're marketing him as a children's hero. He also cannot take any roles that can damage the reputation of the Tony Stark / Iron Man character, i.e no fag / drug addict / criminal roles, etc. Which extremely limits his roles to comedy or kiddie shit and non Oscar bait roles. RDJ said yes but as a return asked to be given % profits instead of lump sum which is rare by kike standards.
