She was a demon. The actress died before they could do anything with that plotline tho so David Chase opted for a more conventional approach, with only a few supernatural elements here and there.

Her husband was a violent mobster who provided her with some reasonably nice things but also embarrassed her publicly by getting caught stealing, as well as abusing her and their kids, shooting a gun through her hair, and abandoning her while she had a life threatening miscarriage to go be with his mistress, a mistress whose whole lifestyle he was financing at the expense of her and their kids. When she sees her fat piece of shit son doing the same things, it understandably pisses her off

Right, Livia sucked but Johnny was a horrible guy, and Tony and Janice inherited all the worst traits from both of them. Her and Junior are such great villains in the first season because you can 100% sympathize with them having to put up with pieces of shit like Tony and Christopher

David Chase hated his own mother irl. He said he didn't want to make the show until after she died.
Even in his 2012 film Not Fade Away, he depicts the mom as insufferable.

Maybe she got sick as a kid and that fucked up her brain? My grandmother was a nasty piece of shit who both physical and mentally abused all her children (even tried killing one them once), she even tried doing it to me when I was younger until my parents cut off contact. We found out years later that she'd gotten sick with something as a kid (can't remember what it was called) and while she seemed like she had recovered it had actually fucked up her brain. Not that it mattered in the end, bitch went into the ground with no-one to mourn her.
