If Tony had died or stayed in the coma longer, who would've became boss of the family? Sil had an asthma attack brought on by the stress of being the acting boss so he's out. Would it have actually been Vito since he was the biggest earner and young and healthy? Paulie has the seniority but lacks the experience and competency.
