My dm uses body part damage. We had a artificer goblin that lost a leg in combat due to him not using proper armor and getting a crit from an enemy. He told us that if we lose a body part, healing would only close the wound but not reattach the part unless we had high level healing magic like regen, we could get an npc cleric to cast it for the party but that was extremely costly. We had our goblin riding shoulders for a while until we modified his steel defender to have a chair and controls to pilot it as a mech and the dm allowed him to always be in the mech at the cost of a permanent spell slot because it was constantly draining his magic to be powered at all times.

Even as someone disabled I don't get the point of this. Admittedly I just have a bum leg, I am not in a wheelchair - but when I play, if I reflect my disability at all in my character it's through characters like a curse-crippped wizard with a big staff, a war vet with a transforming sword cane, or just a regular guy that either got healed or has a funky magic prosthetic or something. Even then I only play characters like this if I can think of a cool story for them where it would fit because I think it's neat and not because I want to see myself reflected in them or whatever.
Why would anyone want to straight up be in a wheelchair? Even if your character is crippled and can't be magically healed for whatever reason, there's way cooler and more interesting ways to work around that than putting your guy in a magic wheelchair. You can go the magic prosthetic route, you could make an old monk who levitates while meditating, you could have a warrior who wears cursed armor that repairs his body but can't be removed, I dunno man. Using a wheelchair seems like the most boring way you could possibly handle that in fantasy.

For me, it's not even that this stuff is in the setting, so much as that they introduce it and then immediately wipe away any negative impact in might have in a medieval society.
Crippled, maimed or otherwise physically disabled? We have magical wheelchairs and prosthetics that are arguably better than being able-bodied.
Playing as a woman? Our medieval society now has complete gender equality.
Playing as a gay character? Our medieval society is also completely tolerant and accepting of LGBTQ+. And as a bonus, we DON'T have any weirdness you might find in history or in the east that might make players uncomfortable. Our gay culture has a perfectly un-problematic and just a mirror image of modern Western gay culture.
And naturally things like racism, slavery, rape, torture and the like don't happen here and they are banned from being mentioned in our campaigns. Nor do you have to struggle with difficult concepts like a race of monsters born into evil.
I just don't understand want this soft, edgeless adult kindergarten bullshit. It's the Care Bears with elves and wizards.

Im the DM for my group, the dumb overpowered magical abilities of the chair are broken, so we wont use them, the chair is a physical object, sure they can use it to move, but ill tell tell them that if they chose to go with the wheelchair route, ill toss up story events that require them to work around the chair, and that many combat encounters will lead to them being left prone, needing a turn to climb back into the chair, and any particularly large hits (like we use for out grievous wounds rules for lasting injuries) can lead to the chair being damages.
cave tunnels that can be crawled through, but, without disassembly, the chair cant squeeze through. Do they take the time, and risk discovery, leave half the party to find another route, or bring the cripple and hide the chair nearby?
rough terrain is a hassle, and will need another party member to carry them, and another the chair.
they WILL get advantages on things that make sense though, the chair will likely take a hit or two for them, they can be moved by another party member without using up their own movement, so a caster can masterblaster their way through combat.
you know, since DnD is a collaborative game, and you can discuss things with the players, and come to compromises.

its a homebrew WoTC advertised yes, but its not hard cannon to the forgotten realms.
but a lot of people i play with are in multiple groups, and almost all of us have horror stories from those other groups, either a new person joins for a while after someone has a kid, or someone moves and they need more players, and that new person 50/50 is a "huge fan of DnD for years, but never played it or read the rules", they put together half thought up characters based on some streamers dnd campaign rules, or bring a laundry list of homebrews they need to play their special character, and more often than not they drop out after 2-3 sessions because its not like their podcasts.
i've been really lucky and in the group i DM most of us are ok with just dropping the game for a month instead of looking for a new player, but in one of the games im a player in we have 3 regulars, the DM and 2-3 "empty seats" where we get in occasional internet folk, if they seem cool to play with they usually end up in the other groups, we mostly run that 'campaign' as a bit of a local group finder/maker.
the freaks are among us, but usually their autism keeps them in the realm of only interacting with the community online, so they never actually PLAY the game much, its why so much of these rules are of the "this is cool" level of thought, with no heed paid to things like balance, or earlygame/lategame progression or even differences.
