He was uncomfortable because his parents specifically told him that they felt bad for doing this to him, that he might've been hurting him, so he felt that he needed to man up and prove his manhood to appease their parents and feel less like a burden. He did this and then felt that it was pointless, the cult was bullshit anyway and he was actually fine with the pretending to be a girl thing anyway, it didn't really feel like pretending to do anything, it just what he behave/dress like.
So every time his gender came up he felt this really strong desire to over correct due to his "feeling bad for his parents feeling bad for him being potentially feeling bad", you could feel it when he did it, his sadness and disappointment. No wonder people treated and viewed him as a girl, he dressed, behaved and looked like one after all. He could change this at any moment, there weren't any lack of role models as he had his brother and a myriad of other bounty-hunters on his journey. He just was comfortable with the way he was, just strung up about his family.
Then he realized that he managed to make everything he could possible do to make his village, parents and himself proud of, not mattering what he looked or behaved like, so he finally ditched this obsession about his gender, and though his journey, accepted that being called a girl is fine. "Uchi wa... onna no ko desu kara!"
The real question is, would Bridget turn out like this if the cult or parents haven't done it?
