It can be done without guides, but you needed to save a lot and keep notes. I beat LSL1-8 without guides, and KQ5 (the only one I ackshually played). Rise of the Dragon was more hairy with the time component, but I still did it sans guide.

Getting the true ending in Steins;Gate
You get random SMS messages throughout the story, and some of them allow you to choose a reply (out of 1-3 options). To get the true ending you must choose the correct reply to 6 or so messages, and it's completely nonobvious which you need to choose.

Being the "best" ending is debatable, but Bloodborne's true ending (and shitty final boss) is locked behind finding three items that, at best, the average player might only find two of.

I doubt many found all three without some googling. The Abandoned Workshop is incredibly easy to miss, so is developing the whore's questline to the end.

Opening the cemetery in Legend of Grimrock 2. You get a small story of a guy traveling around and you somehow are supposed to know that the tundra is north and the desert is south. This also doesn't refer to the game world but most likely from the perspective of Europe. The worst part is that there is a desert in the game, in the north of the island.

Blasphemous hides the true ending behind a breakable wall that doesn't look breakable from the outside, and you need to get there before fighting a boss about halfway through the game or you're locked out of getting it. To be fair, there is another entrance that gets you there later, but at that point you need to restart the game for the true ending. I'm honestly not sure how I feel about it

I remember cross referencing every area map in that game thinking the balance shrine was the mid point intersection between the elemental temple entrances, to going down to every single sublevel of every single shrine, marking where the altar was for each element, extrapolating an intersection point and then relating that to the overworld and all sorts of retarded shit only to find out that the shrine was just where the orbs were
I was pissed.
