Very easily when you run into a super mutant on your way to GNR with a damage threshold it isn't meant to have, with only a 10mm and a hunting rifle with pretty much no AP rounds. It's not a matter of dying, it's a matter of them taking way too fucking long to kill in general without a mod like BLEED/JSawyer.

I played TTW very recently and felt no significant difference between the 3 and NV portions, and I chose to start in the Capital Wasteland. Any problems or imbalances brought from TTW can easily be modded out or toned down to the point of not mattering. Also as another anon said just doing Operation Anchorage early will destroy any semblance of balance faster than any mod ever could.
