I'd say the game is less buggy now than 1,5-2 years ago also, only got 1 gamealtering bug a single time and was able to go back to my load which was a minute prior without issue.
I have also been enjoying the DLC now that I've gotten around to it, it wasn't out during my first round with the game.
The gigs feel a lot more fleshed out, and the story itself was of similar quality to the main story of the game.

My favorite is definitely random car chases throughout the city though, you'll get random gang war shootouts or police chasing down gangs while shooting at them.
It feels a lot more organic now than it did a couple of years ago due to those relatively minor things.
I think optional realistic needs for food/drink/sleep would be cool too, I have literally never used any food or drinks across multiple playthroughs simply because it's a hassle to read what different food or drinks do, and since there is no need for them functionally. Same with sleep.
