It depends on what you want to make. Is the focus on making a video game, with hentai elements? Or is it about making a hentai, with video game elements?

Most RPGM games fall in the latter. They're very much hentais through and through, but with typical jRPG elements sprinkled through, which then causes them to become CG hunting treks. If you take something like Karryn's Prison though (which is also an RPGM game, I know), the gameplay is very much the prominent focus, with the porn being interwoven into it, and this is further evident by being able to play that game with one hand, allowing your joystick in the other hand.

As for gameplay change to revolutionize hentai games for the better, unfortunately the answer is that it would be something subjective, as hentai is in nature. There is no one thing to change. All that can happen is creativity flowing from one creator to the next.

I agree with you though. Some games are just aiming to be good games that happen to have shit you can jerk off to, and some exist to jerk off while using the game part to keep you engaged. The proper use of the latter is to edge with so needs good enough gameplay to not get bored, and properly meted out sexy scenes to keep you going, alongside some user control of when to see them.
