This is triple folded actually, because one last thing not mentioned is direct matchmaking in everything including MMOs, as well as exchanges/general auction houses removed the last real incentive players had to communicate with one another. Trade and grouping.

Keep in mind that sometimes you get situations where people are getting banned or censored for silly things like "K***ht", and appeals to bans are often overseen by political ideologues with a clear bias at best, and at worst obvious mental illness.

I think temporary bans are a sufficient approach to the issue. Hell, you can go a step further and permanently prevent the player from communicating in a multiplayer environment after multiple infractions, that's not beyond implementing. Locking the game entirely is petty overkill and is only asking for trouble.
