They were too rushed to do it. supposedly cutting KotF early on was an act of editorial control, I think what shipped would have been fine with a little more and people would have been none the wiser.

I think it would have worked if -
End chapter 1 with the first credits screen where the Outer Heaven emblem appears.
Begin chapter 2 with the nuke proliferation open; the nuke cutscenes aren't conditional or easy to miss.
End chapter 2 with the nuclear ending and after the monologue, cut back to the mirror and the time jump, specify a date in 1995 and at the end of MG 1 when he plays the tape. End on the final credits where they read the rest of the timeline and end on the line:
Big Boss dies.

It might be possible to do with script in a mod actually. My headcanon is that he's about to make his final stand when he loads the tape and the program on it makes their nuclear stockpile go live. The Boss's pod on Motherbase is the Outer Heaven computer, pulling from the nes version cause why not.

But my headcanon is also that Motherbase is in fact Outer Heaven because that was the original script at various points.
