maruki is a normal dude even after getting his powers, and it's not possible for him to curate and tailor perfect realities for everyone. It starts to slip and he has to set up fully automated "clinics" in mementos/conciousness where people are filtered in and told what to do.
It's hard to notice firsthand because he very specifically tailors the reality of all the phantom thieves to their liking through their councilling sessions, but there's tons of background chatter, dialogues etc as the plot progresses that show he cannot provide the same level as care to everyone.
You have examples like a boyfriend being mindraped into not wanting to study abroad for a dream career by a girlfriend wishing he would stay, A man giving up on a love because another man wished she would love him, people just accepting and being happy with their dead end wagie job instead of striving for more.
Dreams and wishes are incompatible once the sample size increases past a few dozen. There will be thousands of people who want to become big football stars, only a few get that wish granted, the rest have that desire removed.
Joker himself gets mindraped in the ending by Sojiro wished he'd stay with them. Joker, who always wanted to go back to his parents even if he had full choice of not doing so now never wants to leave his new adopted family.
Everything is severely limited by Maruki's ability to process everything and the cracks are already showing.
