The whole game is actually a book written by a depressed cripple that lost his legs after some drunk guy ran over him with his car. His therapist told him that writing something would help him to cope, so he wrote this book where he self inserts into a version of himself that has legs and has to escape a city that is basically Silent Hill, which is what we play in the game.
Also depending of the choice you make in the game it leads to him reaching a certain conclusion about what to do with his depression
he either:
1: Kills himself, his crush and his therapist
2: Kills himself and his crush
3: Kills himself and his therapist
4: Manage to get over his urge to kys, but ends up hallucinating and shooting dead 2 cops that went on a welfare check on his house, then spends the rest of his life a clinic.
