Not like you care, but here's the rundown:
The game plays it off as a joke, Lucas isn't getting raped, he's not even interacting with the guy.
His psychic powers just happened to be awakened a few moments before getting into the Japanese hot spring.
Every time Lucas gets a new ability, he gets a fever. The first time this happens, the player first instinct is to get into the hot spring to heal it.
Japanese hot springs act as healing spots in the game. Think bonfires in Dark Souls.
So, the setup for the joke is that the player has to heal in order to progress, but the only immediate available place is that hot spring with a creepy guy in there.
Now, there's some magical fairy characters in the game that help the main characters, but instead of being some conventionally beautiful beings, they are some ridiculously ugly drag queens, both for comedy and subversion.
Everything surrounding the Magypsies is a tranny joke, that's the real reason that game is never ever getting localized. It would cause an outrage in the west with its depiction of trannies.
TL;DR: Japanese transphobic humor.
