The town of Silent Hill is a lot more run down. Tourism has dried up, small businesses have closed down, unemployment is high, the cult has gotten more powerful, exploiting the general despair and rising superstition of the people to recruit new members.
Eddie is some radicalized Elliott Rodgers, Andrew Tate type, hates women, hates men who are popular with women, killed a couple with a gun he bought on a flea market somewhere and fled to Silent Hill to join the cult.
Angela's basically the same, but ethnically mixed. Half black. Mom, who was black, is dead, dad, who is white, abused and molested her, some additional focus on her brother, now her half brother, also white, being the family's favourite, making her more of a sort of Cinderella, ugly duckling character who's treated worse for her mixed heritage.
It's kind of a gimmick but I guess you could reinvent Maria as sort of a camgirl or OnlyFans model, rather than as a stripper. Wouldn't really make much of a difference, effectively, but you could do something fun and replace the Heaven's Night section with the home of some deranged fan.
Laura's still just Laura. An annoying kid who knew Mary before she died. Guess you could make her slightly older and make her more of a delinquent rather than the brat she was in the original but it doesn't make much of a difference, really.
Other than that... I dunno, add some references to rampant poverty and drug abuse in the apartment levels, the Brookhaven Hospital is more squalid and underfunded, historical society is being pressured by shady government officials to remove any references to the native genocide, the hotel didn't burn down but had to close down because tourism dried up.
