It's a mix of both. I agree that it was executed poorly and a bit confusing, but it was a solid part the story. It's just that the build up was too rushed and it's too subtle for people to understand, especially with how fast pace the plot always move in code geass.
Looking at code geass, think about how the power is activate. It doesn't follow a verbal "command", it simply activate by the user's willl
We got a huge foreshadow that geass can loss control with introduction of Mao, who cannot turn his Geass off. That lost of control is directly linked to his unstable inner state of mind, as his paranoia cause his geass to ALWAYS spies on other people's thoughts.
Then, Lelouche had a huge inner turmoil due to Euphy plan for peace interfere and destroy his plan for revenge and reform of Britannia. People misunderstood some aspect of this struggle. Lelouche did not agree to follow Euphy due to him having a change of heart and "turning good". He agreed to it because he had deep feeling for Euphy (perhaps to an incestous evel of relationship) and couldn't bring himself to kill her. Him accepting Euphy's plan was a massive COPE on his part because he knew it would not work long term and her progress will unravel when she dies of old age or other cause. Him using Geass on euphy to start a massacre is what he planned originally and knew that he had to do to reach his goal. He subconsciously still struggled with his decision until the end and his inner instrusivr thought caused Geass to activate and act against his restraints when the time comes.
