If anything I'd argue that this is a rights holder problem; things like Female V from Cyberpunk were explicitly a choice by CDPR, and supposedly the rights holders get a major say on what comes out. I have severe adhd and the style of mp shooters around the 2000s and early 2010s were perfect for me. Casual jump in, jump out, low investment sessions, being sweaty was optional and an environment you had to willingly participate in. So many mp shooters have released in the past decade and a half that feel amazing to play but god damn do I dislike BR and extraction games. Hate the walking sim part, hate the "looting" and the UI fiddling, I just want to play the game and fight people, get stomped and sometimes do well. I don't care about 5-10 minutes of queues and loading screens after death and then walking for several minutes inbetween fights, I want to go fight right now and mash skulls together. Only games like this left are cod and battlefield and I never liked those IPs so it's grim. It's a personal thing.

Homer has arms and legs that are easier to fit in with little change. Peter is basically a block with short legs, so you can't just put a model of him in as a skin with how Fortnite does them. That said, there was a draft of Peter who still kept the belly. No idea why they didn't stick with that. Maybe they wanted to go all-in on the joke, but it's certainly not aged the best in hindsight.

There's also a chance that they're afraid of introducing too fat a skin in a Battle Pass, since that's their main marketing. Ernie was able to get in with a bigger model, but it clips through shit a lot.
