OSRS is basically Ultima Online. There are three major differences
1) The thousands of quests.
There is so much actual focused content, literal hundreds of hours worth to work through.
2) The grand exchange
Despite whining about it being a popular opinion here, this saved all player interaction from being moved to discord like UO
3) Wildy
Being able to fight and kill players over resources is a huge boon and superbly interesting, a shame this has been toned down

It's pretty much finished, they FINALLY added Menaphos to the map with the last Varlamore update. It's still just a skeleton right now but that's a pretty clear sign they're working toward it, surely they can't fuck it up like RS3 did.

I've been playing it for a couple weeks now and I'm starting to get bored. Everything I want involves doing a bunch of shit I don't want to do and at this point I just ask myself is it really worth it? Sadly theres no other MMO I haven't played to death already so this is all I got.

The thing osrs has that others don't is a wide net of shit to do and actual benefits for it. You can practically play the game however the fuck you want at all levels of play. You can be as lax or as sweaty as you want and there is usually something to fit the mold. Its all about personal freedom goals and interactions and every aspect of the game typically has a subset of players wanting to engage with it to dumbass skills, group based combat activities and even engaging with the economy itself and treating it like a fun stock exchange mini game. Despite what people say about the grand exchange murdering the runescape economy osrs is the only mmorpg with an economy worth engaging with despite its bot issues

I couldn't even tell you what the ffxiv economy is because Gil has been useless for damn near 4 expansions now. If you ever needed Gil for anything its likely someone will give you what you need like candy on Halloween if you are playing woth friends

WoWs economy is a cesspit. You either have to poop sock shit pray you find a niche or roll a mage and farm swipers. The botting is more impactful to the point where they had to change how shit worked in the most recent iteration of classic just to make things remotely functional

Tldr osrs doesn't neglect parts of its players to hyper focus on others short of f2p. Its far from perfect but there is a wide enough variety for it to maintain enough appeal to a variety of players
