The thing is with Slaanesh, is that it doesn't even need to be extreme to start off with, that's usually the middle of it.
Here is an example.
You are walking through the woods, and you trip and fall into a bush. Nothing too bad, you got scratched by a bush, and you go back home and sleep. Next day however, where you fell starts to itch. Your arm is irritated, so you itch it. Feels nice at the moment, and it goes away. A few minutes later, it becomes irritating even more, so you itch it more and a bit longer. Now you're certain you got rid of it, felt good too for some reason. Maybe an hour goes past and suddenly it comes back with even angrier than before, blisters start to show where you itch but you scratch your skin to the point of peeling, it's relieving and strangely feels extremely good, so you keep itching until there is no more feeling, and it goes away. Your skin is bleeding now, but that's alright since you got your itch. Now this repeats for about a day or so every 3-6 hours, you feel irritated, you itch, you feel really good, and it goes away. After the fourth day or so, you have to itch more, the more you itch, the better it feels. Coffee? No need, you got a itch to make you feel better. Sex? Yeah it's good, but when your lover at the time touched where it itched, that felt even better during it. Maybe some weed to help alleviate the itching? Maybe some booze? Turns out, that makes it feel even better, that itch becomes more intense, more pleasure, you don't even realize that you are digging more into your skin with blood oozing out everytime now. After a week of this, people start to notice, you start wondering if they are noticing you are itching your arm more and more, and they get concerned. Who cares, that itch is good feeling when you scratch, and it's always nice. Suddenly it stops. That itch you were had just stops. You wonder why it stopped, but maybe it's for the best. continued
